Law Society of Ontario (LSO) Lawyer or Paralegal Licensing Process Application

Any person wishing to practice law as a lawyer or a paralegal in Ontario must complete the Lawyer or a Paralegal Licensing Processing to be licenced by the Law Society of Ontario (LSO).

You are required to submit a completed and commissioned application along with certain notarized documents in order to apply to the lawyer or paralegal licensing process and allow you to write licensing examinations.

Ontario Lawyer or Paralegal Application for Surrender of Licence to Practice Law Under By-Law 4

Licensees who no longer wish to provide legal services in Ontario may apply to the Law Society of Ontario (Law Society) to surrender their licence. Applicants are encouraged to review By-Law 4 for information about surrendering their licence.

Statutory Declaration for College of Registered Psychotherapists and Mental Health Therapists of Ontario (CRPO) Application

To become a registered psychotherapist or mental health therapist in the province of Ontario, you must submit an application to the College of Registered Psychotherapists of Ontario. A commissioned statutory declaration must be completed and included in your application package to the CRPO.

Royal College of Dental Surgeons of Ontario (RCDSO) Certificate of Registration Application

In order to practice dentistry in Ontario, you must be registered with the Royal College of Dental Surgeons of Ontario (RCDSO). 

To register with the RCDSO you must apply for a certificate of registration (licence) by submitting an application. Your application must be  be commissioned before a notary public.

College of Dental Hygienists of Ontario (CDHO) Application to Become A Registered Dental Hygienist in Ontario

In order to become a Registered Dental Hygienist in the Province of Ontario you must submit an application to the College of Dental Hygienists of Ontario (CDHO) to obtain a certificate of registration. Documents that are part of your application package may need to be notarized, including Criminal Record Check,  Identification documents demonstrating that you are currently eligible to work within Canada (Canadian birth certificate, passport, citizenship certificate, permanent resident card, or valid work permit), and notarized copy of documentation to support a name change, if applicable.
