You must have two valid government-issued pieces of identification with a photo and your address. ]

Examples of acceptable ID include, Provincial Driver’s License, Passports, Permanent Resident Card, Citizenship Card, First Nations Card, Age of Majority Cards (e.g. the Ontario Photo Card), Health Card (as a second piece of ID only), Government Employee Photo Cards, etc.

Do not sign or date your affidavit in advance as it must be done in the presence of the notary public.

OSAP affidavit is a legal document that is submitted along with an application for financial assistance to OSAP (Ontario Student Assistance Program). What is Affidavit of Service-Form B?
An Affidavit of Service (Form 16B), also known as Proof of Service, is a legal document that serves as proof that someone received a legal document from another party.
What is an Affidavit of Identity?
An Affidavit of identity is a legal document that lets a person attest to their own identity and verify own signature. It is typically used when your name is spelled incorrectly on your identification documents, like driver's license,  passport, permanent resident card, birth certificate, marriage certificate etc. An Affidavit of identity will confirm that different names on your identity documents refer to you as as one and same person and may be used for various purposes such as employment, education, immigration, banking, tax purposes. 
What is an Affidavit of Name Change?
If you wish to change your name as an adult or change a name for your child either due to marriage, divorce, by deed poll, or court order, you need to fill out an application form and submit it to Service Ontario to receive a change of name certificate. Here are two applications that are used: Application to Change Name in Ontario (Form 11155E) or Application to Change a Child's Name (Form 11156E) and must be signed in the presence of a commissioner.

What is an Affidavit of Vehicle Ownership without Bill of Sale?
If you purchased a vehicle in Ontario but did not receive a bill of sale, vehicle ownership or it if was lost or destroyed, you will need to provide a notarized affidavit of ownership with details of purchase and reasons for not providing a bill of sale for registering your vehicle with Service of Ontario.
